Diablo 4 Malignant Hearts List & How They Work

Card No. 06·26505 · Updated · Standard formula, shown below

Malignant Hearts are socketable items in Diablo 4 Season 1 (Season of the Malignant) that grant powerful legendary-style effects. You insert them into special sockets on your jewelry (rings and amulets) after defeating a Malignant enemy and using a Cage of Binding. This article lists every Malignant Heart, explains how to get and use them, and answers common questions about the seasonal mechanic.

What Are Malignant Hearts?

Malignant Hearts are a seasonal mechanic introduced in Diablo 4’s first season. They function similarly to gems but provide unique, build-defining powers. Each heart is color-coded into four categories: Vicious (offensive, red), Brutal (defensive, blue), Devious (utility, yellow), and Wrathful (powerful, black). You can only socket a heart into a jewelry piece that has a matching colored socket. Wrathful hearts are special: they can fit into any colored socket, and they are the rarest and most powerful.

Hearts are obtained by defeating Malignant Monsters, which spawn from corrupted enemies, and then using a Cage of Binding to extract the heart. The heart’s type and power are random, but you can craft specific types using the Malignant Invoker at a workbench.

Complete List of Malignant Hearts

Here is the full list of Malignant Hearts in Diablo 4 Season 1, organized by color. The names and effects are accurate as of the season’s release.

Vicious Hearts (Offensive)

  • Picana – Critical Strikes electrically charge the enemy for 3 seconds, causing lightning to arc between them and other charged enemies.
  • The Lionheart – You are Unstoppable while at 100% Life for 4 seconds after taking damage.
  • The Barbers – Critical Strikes and all subsequent damage within 2 seconds are absorbed into the target, then erupt outward dealing 10-20% of the absorbed damage to the target and nearby enemies.
  • The Grandfather – Lucky Hit: Up to a 100% chance to make enemies Vulnerable for 2 seconds.

Brutal Hearts (Defensive)

  • The Sacrilegious – Automatically activates an equipped Corpse skill every 1-2 seconds. (Necromancer-specific)

  • The Ravenous – Lucky Hit: Up to a 20% chance to gain a 50% chance to execute non-Elite enemies.

Devious Hearts (Utility)

  • The Malignant Pact – Every 20 seconds, your next attack is guaranteed to be a Critical Strike, Overpower, or Lucky Hit, cycling in that order.
  • The Decrepit Aura – When at least 5 enemies are near you, gain an aura that automatically applies Decrepify to them. (Necromancer-specific)
  • The Frozen Terror – Lucky Hit: Up to a 20% chance to freeze enemies for 2 seconds.

How to Get Malignant Hearts

To obtain a Malignant Heart, you must first find a Malignant Monster. These appear randomly in the world, often from corrupted enemies that spawn during the seasonal questline. When you defeat a Malignant Monster, it drops a Malignant Heart (the item) and a Cage of Binding (a consumable). You then use the Cage on the heart to extract it into a socketable form.

There are also Malignant Tunnels, special dungeons that guarantee Malignant encounters. At the end of a tunnel, you can use a Malignant Invoker (crafted from materials) to summon a specific type of Malignant monster, which drops a heart of that color.

How to Socket and Use Malignant Hearts

Malignant Hearts are socketed into jewelry (rings and amulets) that have a Malignant socket. These sockets are colored: red, blue, yellow, or a universal white socket that accepts any heart. You can find jewelry with Malignant sockets as drops or by using a Malignant Socket item on a regular piece of jewelry at the jeweler.

To socket a heart, open your inventory, right-click the heart, and then click the socket on your jewelry. You can remove a heart at any time, but it is destroyed in the process. You cannot salvage hearts for materials, but you can sell them to vendors.

How to Choose the Right Malignant Heart

Choosing a heart depends on your class and build. Here are some general guidelines:

  • Offensive builds – Look for Vicious hearts like The Barbers or The Grandfather to boost damage.
  • Defensive builds – Use Brutal hearts like The Dark Dance for survivability, but be aware of the life cost.
  • Utility builds – Devious hearts like The Malignant Pact provide consistent procs.
  • Endgame – Wrathful hearts are the strongest, but also the rarest. If you find one, build around it.

Also consider the class-specific hearts. For example, a Necromancer will benefit greatly from The Sacrilegious (auto-corpse skills) or The Great Feast (essence drain for damage).

Common Problems and Misconceptions

Problem: Hearts not dropping. Malignant Hearts only drop from Malignant Monsters, not regular enemies. Make sure you are fighting the correct enemies and completing the seasonal questline to unlock the mechanic.

Misconception: Wrathful hearts are class-specific. While some Wrathful hearts have class restrictions, others like The Ouroboros are universal. Always check the tooltip.

Problem: Cannot socket a heart. Your jewelry must have a Malignant socket of the matching color. If it doesn’t, you need to apply a Malignant Socket item first.

Misconception: Hearts are permanent. Malignant Hearts are only usable during Season 1. After the season ends, they will not transfer to the Eternal Realm. The mechanic is seasonal-only.

Malignant Hearts vs. Legendary Aspects

Malignant Hearts are similar to Legendary Aspects in that they provide powerful effects, but they occupy a different slot. You can have both a Legendary Aspect on your gear and a Malignant Heart in your jewelry, so they complement each other. However, hearts are limited to jewelry, while aspects can go on any slot. This means you must choose which jewelry piece gets the heart versus an aspect, as you cannot put both on the same item.

Frequently Asked Questions

Can I use Malignant Hearts in the Eternal Realm?

No. Malignant Hearts are a Season 1 exclusive mechanic. After the season ends, they will not work in the Eternal Realm, and the seasonal sockets will likely be removed or become inactive.

How do I get a Wrathful Malignant Heart?

Wrathful hearts drop randomly from any Malignant Monster, but at a very low rate. The most reliable way is to use a Wrathful Malignant Invoker at the end of a Malignant Tunnel, which guarantees a Wrathful heart drop. The invoker is crafted from materials dropped by Malignant elites.

Can I remove a Malignant Heart and reuse it?

No. Removing a Malignant Heart destroys it. You can only extract it once, and it cannot be re-socketed after removal. Plan your socketing carefully.

Do Malignant Hearts work in World Tier 4?

Yes, they work in all World Tiers. However, the power of the hearts is fixed, so you may want to upgrade your gear around them as you progress to higher tiers.

Are Malignant Hearts better than regular gems?

For most builds, yes, Malignant Hearts provide more impactful effects than a simple stat boost. However, some builds might prefer the raw stats from gems, especially if the heart’s effect doesn’t synergize with your skills.

Can I trade Malignant Hearts with other players?

Yes, Malignant Hearts are tradeable, as are the Cages of Binding and Malignant Invokers. This allows you to buy or sell specific hearts you need.
